Leeds United avoided loss with a backheel from substitute Perkins in the 93rd minute, sending the game to a rematch with a 2-2 tie.
Cardiff City, who are 20th in the Championship division, led by two goals via Jaden Philogene and Sheyi Ojo and appeared to have practically sealed their ticket in the fourth round of the FA Cup, but regrouped Leeds responded with two goals in the second half to send the game into a replay.
Leeds would have scored three goals if Rodrigo had converted his penalty after Joel Bagan was sent off for handball in the box.
Cardiff appeared to have finished the mission and their tenacious defence looked to have done the job, but Perkins saved the Yorkshire with a clever backheel goal in the 93rd minute to spare Jesse Marsch's heartbreak.

Despite Leeds control of the game, it was Cardiff who took the lead in a run of play when Tom Sang provided a through ball to Mark Harris whose fine shot was saved by Joel Robles, but the deflection came towards Harris who teed up Isaak Davies, and Joel was once again quick to deny Isaak before ball fell to Philogene who rifled from close range.
Cardiff fans were enjoying every touch of their players and looked optimistic as they were on course to fourth round of FA Cup, but it was visitors fans who had the last laugh as Perkins equaliser from Firpo’s flick sparked jubilant mood from the travelling fans.
It would have been their eighth defeat in all competitions if they had lost, but this victory has undoubtedly removed some of the American manager's strain.
This victory should have boosted their confidence, and the players will seek to build on it, as Perkins assisted Firpo's flick into the net, sparking wild celebrations and forcing a replay.
